How to apply for your UMID: a simple guide

I just finished processing my application for getting my Unified Multi-Purpose ID (UMID) today. Although it took me for more than 2 hours including the waiting time, I think that it was still very easy and fast in getting it. These are the 5 simple steps you can follow in your application:

Step 1. Go to your nearest SSS branch in your area.

As for me I went to Butuan branch as went there as early as possible since SSS office is one of those offices that people are visit a lot.

Step 2A: Get the UMID form from the counter 1 if you have not downloaded beforehand.

2B: Download the form here: and directly go to counter 2 for the priority number.

Step 3: Pass your filled out form and present 1 valid ID to counter 2 for encoding

I presented my passport ID. After you get an approval of your form from counter 2 you can now proceed for bio-metrics and photo capturing

Step 4 : Bio-metrics and photo capturing

You need to have your thumb and right finger for both left and right electric thumb scanning , affix your digital signature and then ready yourself for photo .

Step 5: Get your claim form and wait for 3 months as per advice and claim your ID in counter 1.
